On ADDITIONAL INFORMATION on product´s page, is it possible to bring some customizable form fields from BLING (the front desk software we integrate with Woodmart)?
If not, I ask you: not even with a programmer professional?
Try to create HTML Blocks with the page builder and create form as you want > Then Go to Theme Settings > Single Product > Tabs > Additional Tabs > enter the tab title > select the content the type HTML blocks that you have made.
