Missing checkbox for different shipping address

    Hello,
    I have 2 issues on my site:

    1. when you select on the checkout to pay with credit card, everything is not looking good there. please let me know how to fix it.
    2. on the checkout there is a missing checkbox to “ship to a different address”. I tried to disable all the plugins, to set the billing to the user but nothing help. if I am disabling your theme and using a default theme, it is shown.

    Please let me know how to fix it ASAP.
