Subscription Form Not Working

Dear Support,

I have key in my MailChimp API.
I wanted to add in First Name, Last Name and Date of Birth.

However, when I click the Add more Fields, it doesnt have links to click (Add list fields to any of your selected lists. Clicking the following links will take you to the right screen)

I tried to put in some codes at my own and the form cant submit, with the reason “Please select at least one list.”

Please advise me on this soonest possible.
