Tracking order form empty

When I go to tracking order page , the tracking order form is empty, when can only see green track button like the screenshot below.
Please tell me if is is normal ?

I saw the screenshot you attached. It seems that you are using two pages for the track order as you can see in your screenshot.

Please Go to Dashboard >> Pages. Delete the track order duplicate page. Then make sure by editing the page that the module is added in it.

Clear cache and check back.

I delete duplicated order track page and cleared cache but as you see on 2 screenshots it is same .

I saw the screenshots you attached. Please Go to WoodMart >> Additional Pages. Select track order page from the dropdown.
Then import the page.

Then you can use the page as the track order page.
