Review Email and Name not shown

Hi
I have noticed that in product reviews tab there is no email and name field available but there are available in demo.here just rate(star) and message are shown.
in dashboard > settings > discussion > checked true: comment author must fill out name and email.
I have reset theme to default and no luck.
Can you help me what’s wrong about reviews?

Please check how it works with the default wordpress theme just to check whether the issue is with our theme or not.
Because the reviews are woocommerce functionality and our theme doesn’t influence.

Should i reinstall woocommerce?
Or may i replace plugin file from host with ones included in package?

As it works the same with the default wordpress theme which clears that the issue is not from our theme. Sorry but we could not help you with this. You can try asking the woocommerce support for help.
