Adding multiple choice boxes and message boxes to improve customer experience

Hi. I would like to add a section where the customers who visit my site can mark the ready options with the checkbox and write a message in order to evaluate their experiences. If possible, either a random code snippet (EXAMPLE = AP256Z) or a structure with an email address added to separate each evaluation from each other. But I don’t have any coding knowledge, can you tell me how can I do this?
I am sharing the link of a structure similar to what I want.
