Blank box appearing everywhere on top of contact forms

Could you please guide me about how can I remove the blank box which can be seen in the attached screenshots. Please also provide the reason for future reference / projects.

Please add the below Custom CSS code to Theme Settings > Custom CSS > Global Custom CSS:

.wpcf7 .hidden-fields-container {
    display: none !important;
}
