Contact form broken

Dear Support,

currently my contact form broke for unknown reasons, the elements are all next to each other rather than grouped in 2 in each row.

There should be two elements in each row only the radiobox and the last big textfield should be alone:

…
radiofield
bigtextfield

Nothing I do changes this layout
