Element form hiding fields

Hello, I made a contact page and put a simple form on it, on the edit page everything is fine, if I go to see the page later, some fields are disappearing, and consequently customers can’t get in touch, I’ve tried to redo the page and the problem persists, can you help in this regard?

Please check how it works with the default wordpress theme just to check whether the issue is with our theme or not.
Because you are using the form element of Elementor and our theme doesn’t influence.
