Form not working on After "Add to cart button" text area

I have deactivated all plugons and still not working.

Ive cleared cache and try it on both logged and private sessions.

The form works well now. Do you mean that it doesn’t work with your customizations made?

The form is not woring.

It does not displays all the times nor on different explorers and type of sessiones
