How to add Agreement to Terms of Service and Pricavy

When creating an account, it is required to have the agreement of Terms of Service and Privacy Statements from the customer. However, we only mention ‘privacy policy’ at the register stage without a check mark. (Please see attached pic ‘Current’)
Can we have a check mark radio button for ‘Terms of Service’ & Privacy Statements’ at the resister page which is required section? (Please see attached pic ‘Example’)

Hi Thanks for your reply. We just extended the support from you guys.
We installed the plugin called ‘WP User Manager’ and it creates the new register page automatically with the checkbox of Terms and conditions & Privacy Policy.
And we want to change the default page of register to the new page when clicking ‘Account’ icon. However, we do not know how to do it.
How can we change the default register or account or log-in pages?
