Reviews are the option to estimate a product and leave a comment. You can enable the Reviews in Woocommerce > Settings > Product.

Hello,

You can disable attaching a photo. You can show a review option in a tab or in a separate section. All these options are provided in the Theme Settings > Product page Reviews / Tabs.

At the same time, it is not possible to add the comment field exactly as you show. Perhaps you will have to find a plugin.
