I saw the screenshot you attached.

Please add form HTML in as the following under Dashboard >> MC4WP >> Form:

<p>
	<label>Email address: </label>
	<input type="email" name="EMAIL" placeholder="Your email address" required />
</p>

<p>
	<input type="submit" value="Sign up" />
</p>

Then clear cache and check back.

Please Go to Dashboard >> MC4WP >> Forms and enable the option.
