Shortcode insert

When I create a page and insert a specific shortcode, the default layout breaks like the image I attached. Did I set something up incorrectly, or are the settings wrong? The plugin I’m using is Coupon Affiliates for WooCommerce.

The page looks normal when it first loads in a different web browser, but after I enter something and submit it, the redirected page appears broken like in the screenshot. Do I need to configure something separately? If possible, could you please try creating a test “Register New Affiliate Account” once?
