Spam User Registrations

Hi,
I am getting a lot of spam user registrations. In the ‘Register’ option, all you need is the email and once ANY email is input, the user can go into the Accounts page and set up details.
Is there anyway to stop this? How can I add additional fields/recaptcha etc on the Register page? And require that users confirm email-ids (by clicking on a link in the email) before displaying the MyAcccount page? I am spending a lot of time deleting spam users. Please help
