Unable to upload image

Whenever I try to upload an image, it will tell me upload failed. and when I try to add image thru the image gallery, the loading button will just spinning forever. Please advise on any possible solutions.

Could you please check how it works with some default WordPress theme like twentysixteen to understand is it our theme issue or not?
