Widgets not responding in footer

Using a custom plugin that is enabled by “[widget]” in the footer area and homepage. The widget correctly works on the homepage, but when used in the footer the send function just refreshes the page.Something in the footer is not allowing the widget to work correctly.
Location: “TEXT MESSAGE OUR INFO TO YOUR PHONE” on home page (works!)
Footer Q4 (does not work!)
Short-code: “[widget]”
