Hello,
1. Create the template you want to add in the Dashboard > HTML
2. Add a contact form into a popup element https://xtemos.com/docs/woodmart/shortcodes-2/
3. Then add the shortcode to the Theme Settings – > Product page – > After “Add to cart button” https://xtemos.com/docs/woodmart/html-blocks-2/#before_after
