As per screen provided, you are talking about Popup element which contains Contac form. Edit the page http://prntscr.com/kz9696
Check which contact form has been chosen http://prntscr.com/kz97jj
Navigate to the Contact Form plugin > Contact forms http://prntscr.com/kz984z
