Here are steps to add mailchimp form to your site footer

1. Install and activate mailchimp plugin https://wordpress.org/plugins/mailchimp-for-wp/

2. Create a newsletter form

3. Copy it’s shortcode and place into footer widget column in Appearance -> Widgets
