If you don’t see this form on the checkout page then you didn’t configure your shipping correctly in WooCommerce. We suggest you to read plugin’s documentation for more information about this since it is not our theme related issue https://docs.woocommerce.com/document/configuring-woocommerce-settings/#section-9

As we can see, you didn’t set up any shipping method and shipping zone in your WooCommerce settings. Please, do this firstly. Read the plugin documentation if you have any difficulties with that.
