contact form on product page

I want to put a contact form for my out-of-stock products in my Woodmart theme. It is not in stock, but you can contact us for bulk purchases, I want a button etc. Do you have any suggestions for this? I can also use this form for all my products. The URL of the website I developed is as follows.
