how to put a contact form on the homepage like this?

The screenshots show the Newsletter subscription popup. This block has been created in HTML by means of using background images, Section title and the shortcode for the newsletter from http://prntscr.com/kkhhpt

You can take popup template and use it for your contact form changing the shortcode with the form for Contact form element.

Save the template in HTML block, insert it into the page, drag and drop to the place as per your needs.
