Phone field issue with Back in stock notifier plugin

Hi
I use Back in stock notifier plugin on my website, and for some reason the phone field is not displayed correctly.
The theme removes the field attributes, such as placeholder and country code field.
When I change to a defualt theme, the form is displyaed correctly, but then switching back to Woodmart (parent theme with reset option enabled) – the field is not displayed correctly.
I already contact the plugin support team but they said there is nothing wrong with the code, and now I checked it on a staging environment and I notice the theme is causing this issue.
I’m not sure what in the theme is causing it, I will be glad to see if you can help to figure this out.
