What exactly code do you mean? The form is displayed correctly and it doesn’t depend on our theme. Please, check how it works with default WordPress theme like twentysixteen. You can create a page with a shortcode of the mailchimp form [mc4wp_form id="2528"] and test it. Write us about the result.

The issue is not Mailchimp, I can’t test the popup on another them.
I just copied your code + the code [mc4wp_form id=”2528″] in the Promo popup.
