How to make a check out page like this?

I know I must create a custom layout for check out, but what specific elements/structure can I use within WP Bakery to make the form look like this?
Any tips/guidance would be much appreciated.

Hello,
For the checkout design, you can use our predefined layout and check the preview of the layout.
That box should not be normally checked by default. You can configure it from WooCommerce > Settings > Shipping > Shipping options.
