Short code not working

I have a issue related to the shotcode
When I’m using short code in my Quick Order Form Page it’s just show the empty page .
Second.
Password forgot page not find
When I’m click on password forget show 404 error page
So now tell me how to fix it.
I hope you understand my issue.

1. Please elaborate what issue you are facing with the help of some relevant screenshots.

2. Please check how it works with the default wordpress theme because my account is woocommerce functionality and our theme doesn’t influence.
