Unable to upload media files

Hi,

I am unable to upload any media files. The upload button does not seem to work. I tried to use the browser upload feature in the Media section to upload a few, but it does not always work, What could the issue be?

Initially I had trouble with uploading product images. now when i am trying to upload media in the widget area in header, the add media button is not functioning.

please help.

Could you please check how it works with some default WordPress theme like twentysixteen to understand is it our theme issue or not?
