Have to login twice

I have to login twice on my site. When I go to Login page and fill put my details and submit the form it just takes me back to the same page.

There is no error, no warning/errors in console and nothing in Error console.

After that happened, if I fill my details again and submit; I am logged in.

This is really frustrating.

Can you please let me know how can I fix this.
